FEE SCHEDULE: All fees include use of all necessary equipment, except spikes (screw in spikes are not allowed), and athletic supporter. All players will be given a high quality Bike game jersey imprinted with their name to keep. FEES ARE NON-REFUNDABLE!!!. The fees are as follows:

	INDIVIDUAL: $175
	EACH ADDITIONAL CHILD: $125
	AFTER JUNE 30th: ADD $25

VOLUNTEER EFFORTS

Volunteer efforts are an integral part of our organization. Through volunteers we are able to maintain a high quality program for your child, and keep our costs within reason. Please indicate below in which area of the organization you would be willing to assist.

Terms and Conditions: In return for my/our child being permitted to participate in the sport of Football, a voluntary activity undertaken by the Bulldog Youth Football Club, during Fall 2008, and understanding that there are certain risks both in connection with such activity and in traveling to and from such activity, and intending to be legally bound hereby, EACH OF THE UNDESIGNED, for himself/herself an on behalf of our child, as well as his/her heirs, assigns and legal representatives hereby expressly agrees to: 1. RELEASE, WAIVE, DISCHARGE AND AGREE NOT TO SUE, Bulldog Youth Football Club and all its successors, assigns affiliates, officers, directors, coaches employees and agents (''Bulldog Youth Football'') from all manor of actions and causes of actions, suitsdebts, accounts, judgments, claims and demands whatsoever in law or equity, and attorneyfees, including all claims arising out of any incidents involving personal injury in any way by reason of participation in the activity; 2. I ASSUME ANY AND ALL RISKS involving in or arising from our child's participation in the voluntary activity, including without limitation, the risks of death, bodily injury or property damage, the unavailability of amount of emergency medical care or the negligent or deliberate act of another person; 3. INDEMNIFY, DEFEND AND HOLD BULLDOG YOUTH FOOTBALL and its officers, directors coaches employees and agents harmless from any and all claims, causes of action damages, judgments costs or expenses including attorney's fees whatsoever from our child's participation in the activity. 4. ASCERTAIN WHETHER MY/ OUR CHILD HAS ANY HEALTH CONDITION which make it inadvisable for him/her to participate in the voluntary activity. 5. ASSUME ANY AND ALL RESPONSIBILITY FOR THE RETURN OF EQUIPMENT provided to my child and agree to reimburse the Bulldog Football Club for any and all equipment not returned at the end of the season. My signature below indicates that I have read all of the above material and fully understan this Waiver as well as the risks, hazards and stresses as the apply to participation in this voluntary activity and that by signing this I/We surrender valuable rights, which is done freely and voluntarily.
